The form below applies to benefits that we pay directly to you. It can be used to claim for any of the following:
Accommodation & Travel benefit – available when a patient travels for out-patient chemotherapy, radiotherapy or cancer related immunotherapy and/or is required to stay overnight in accommodation convenient to the hospital, where you travelled more than 50km
Travel by public transport, taxi, hackney or petrol, diesel or electric vehicle charging for your car where you travel more than 50km
Car parking costs, incurred as a result of out-patient chemotherapy, out-patient radiotherapy or cancer related immunotherapy
Details of the hospital attended for treatment is required (must correspond with a Vhi Hospital Claim on file or be supported by hospital documentation confirming treatment dates).
Nursing care at home (for a member under 18 years of age at last renewal date) following a stay in hospital of 5 days or more.
Benefit is payable for a specific period toward the cost of the accommodation. Here is the Convalescent Claim Form.
Benefit towards the accommodation costs of a family member as detailed on relevant plans.
Travel and accommodation expenses for parent/guardian accompanying a child (under 14 years at last renewal date) during that child's hospital stay in excess of 3 days.
Home nursing, by a registered nurse, incurred within 3 days of delivery.
Available following a hospital stay of 5 days or more on relevant plans.
